What medical records do I need to bring to my DOT physical examination?

Med­ica­tions

A com­plete list of ALL med­ica­tions; pre­scrip­tion, over-the-counter, herbal reme­dies, diet sup­ple­ments includ­ing dosage and doctor’s name. Med­ical records and a note from your treat­ing clin­i­cian regard­ing your safe­ty while oper­at­ing a CMV might be required.

Eye Glass­es / Con­tact Lens / Hear­ing Aids

If required to wear, must be brought to the examination.

Insulin-Treat­ed Dia­betes Mellitus

Must have MCSA-5870; Insulin-Treat­ed Dia­betes Mel­li­tus Assess­ment Form com­plet­ed by treat­ing doc­tor with­in 45-days of your DOT physical.

Non-Insulin Treat­ed Dia­betes Mellitus

Results of a Hemo­glo­bin A1C with­in past 4 months.

Results of a dia­bet­ic eye exam with­in past 12 months.

Copy of recent med­ical eval­u­a­tion from your health­care provider out­lin­ing your his­to­ry, treat­ment and con­trol, and any com­pli­ca­tions such as kid­ney, eye, or nerve damage.

Heart-Relat­ed Issues (e.g., Stents, Valves, Bypass, Surgery, cur­rent med­ica­tions, treat­ment, etc. Heart Attack, Arrhyth­mia, etc.)

Copy of records from car­di­ol­o­gist out­lin­ing your history.

Results of most recent of any EKG, Stress test, echocar­dio­gram or oth­er testing.

Sleep Dis­tur­bances (Obstruc­tive Sleep Apnea)

Copy of most recent records from your sleep specialist.

Most cur­rent 90-day report from your machine.

Seizure Dis­or­der / Stroke / TIA

Copy of records from neu­rol­o­gist out­lin­ing your his­to­ry, cur­rent med­ica­tions, neu­ro­log­ic and psy­chi­atric state.

A URINE SPEC­I­MEN IS REQUIRED FOR THIS PHYSICAL

You might be required to pro­vide addi­tion­al doc­u­men­ta­tion as deter­mined by the Cer­ti­fied Med­ical Exam­in­er.
