Care Management


Care Man­age­ment Ser­vices at Quin­cy Med­ical Group offer pro­grams to help patients with chron­ic con­di­tions improve the over­all qual­i­ty of their lives, ensure that they receive care based on the best med­ical prac­tices, improve their under­stand­ing of their con­di­tion, and reduce hos­pi­tal admis­sions and emer­gency depart­ment vis­its. The care man­agers are an exten­sion of the pri­ma­ry and spe­cial­ty care teams and will work with the patient, their fam­i­ly, nurs­ing staff, and their pri­ma­ry care provider.

A Nurse Care Man­ag­er will:

  • Assess patient
  • Edu­cate patient on dis­ease process
  • Pro­vide med­ica­tion training/​monitoring
  • Assess for lifestyle modification
  • Rec­om­mend self-care measures
  • Assist in client/​provider communication
  • Refer to com­mu­ni­ty and med­ical resources
  • Pro­vide reminders for appoint­ments, screen­ings, and tests
  • Pro­vide reg­u­lar mon­i­tor­ing calls
  • Pro­vide com­mu­ni­ty referrals

Peo­ple who iden­ti­fy with the fol­low­ing state­ments can ben­e­fit from Care Management:

  • Ongo­ing com­pli­ca­tion­s/­co-mor­bidi­ties
  • Fre­quent ED visits/​hospitalizations
  • Lack con­fi­dence in self-care
  • Lifestyle or behav­ioral risk factors
  • Psy­choso­cial, envi­ron­men­tal, or health­care access risk
