Community Healthcare Survey Terms & Eligibility

You do not need to be a Quin­cy Med­ical Group patient to com­plete this sur­vey. The sur­vey takes about 6 – 8 min­utes to com­plete. The first 200 qual­i­fied respon­dents who com­plete the sur­vey are eli­gi­ble to receive a $10 HyVee gift card.

Incen­tive Eligibility

Qual­i­fied respon­dents who com­plete the sur­vey may receive a $10 Hy-Vee gift card, sub­ject to the fol­low­ing terms:

  • The incen­tive is avail­able to the first 200 qual­i­fied respon­dents who ful­ly com­plete the survey.
  • Respon­dents must be 18 years of age or old­er and live in Illi­nois, Iowa, or Missouri.
  • Employ­ees of Quin­cy Med­ical Group, Duly Health and Care, and their affil­i­at­ed orga­ni­za­tions are not eli­gi­ble to receive an incentive.
  • Lim­it one incen­tive per per­son and per household.
  • Respon­dents must pro­vide accu­rate con­tact infor­ma­tion and a valid mail­ing address for gift card delivery.
  • Incom­plete, dupli­cate, fraud­u­lent, auto­mat­ed, or oth­er­wise invalid sur­vey respons­es are not eli­gi­ble for an incentive.
  • Respons­es that fail sur­vey qual­i­ty checks or do not meet the study’s eli­gi­bil­i­ty require­ments may be excluded.
  • Eli­gi­ble respon­dents will receive one $10 Hy-Vee gift card, while sup­plies last, up to a max­i­mum of 200 gift cards.
  • Gift cards will be mailed direct­ly to eli­gi­ble respondents.
  • Gift cards are sub­ject to Hy-Vee’s applic­a­ble terms and con­di­tions and are not redeemable for cash through Quin­cy Med­ical Group or Duly Health and Care.
  • Par­tic­i­pa­tion in the sur­vey is vol­un­tary, and eli­gi­bil­i­ty for the incen­tive is not depen­dent on pro­vid­ing any par­tic­u­lar opin­ion or type of response.
  • The sur­vey spon­sor reserves the right to mod­i­fy, sus­pend, or end the incen­tive offer if nec­es­sary due to tech­ni­cal issues, sus­pect­ed mis­use, fraud, or oth­er cir­cum­stances affect­ing the integri­ty of the survey.

Pri­va­cy Notice: Con­tact and mail­ing infor­ma­tion col­lect­ed for gift card ful­fill­ment will be used only to ver­i­fy eli­gi­bil­i­ty and deliv­er the incen­tive. This infor­ma­tion will not be used as part of the sur­vey analy­sis or com­bined with sur­vey respons­es for report­ing purposes.
